A study of nipocalimab in participants with active idiopathic inflammatory myopathies

Inflammatory muscle diseases, known as idiopathic inflammatory myopathies, are rare conditions where the body's defence system mistakenly attacks healthy muscles. This causes ongoing muscle weakness, joint pain, and severe tiredness. This trial is testing an experimental medicine called nipocalimab. Nipocalimab works by lowering certain proteins in your blood called antibodies, which are thought to drive inflammation in your muscles. Participants will be placed into one of two groups: one receives nipocalimab through a drip into a vein, and the other receives a dummy medicine (placebo). Neither you nor your doctor will know which one you receive during the first year. Later in the study, all eligible participants can receive nipocalimab. Taking part lasts for up to 112 weeks and involves regular clinic visits, blood tests, and health checks to see how well the treatment works and monitor your safety.

यह अध्ययन किस बारे में है?

Idiopathic inflammatory myopathies are a group of rare, long-term conditions that cause painful inflammation and weakness in the muscles we use for moving. People living with these conditions often struggle with daily tasks, such as climbing stairs, lifting objects, or getting out of a chair, alongside extreme tiredness and painful joints. At the moment, treatments are limited and do not work well for everyone.

In these conditions, the immune system produces harmful proteins known as antibodies that attack the muscles. Researchers have developed a medicine called nipocalimab to tackle this. Nipocalimab works by blocking a specific receptor in the body that normally keeps antibody levels high. By turning down these antibody levels, doctors hope the medicine will cool down inflammation and give muscles a chance to recover.

This study aims to discover whether nipocalimab can successfully reduce symptoms compared to a dummy treatment, called a placebo. Because this is a Phase II trial, researchers also want to learn how safe the medicine is and understand the best way to use it in the future.

मुख्य निष्कर्ष

  • Tests a new medicine, nipocalimab, for active inflammatory muscle disease.
  • Open to adults aged 18 and older with specific blood test antibodies.
  • Compares nipocalimab against a dummy drip (placebo) during the first year.
  • Eligible participants can receive the real medicine in the second year.
  • The total study duration is up to 112 weeks, with close medical monitoring throughout.
  • Participation is entirely voluntary, and travel costs are reimbursed.

कौन पात्र हो सकता है?

This trial is for adults aged 18 and older who have an active, diagnosed inflammatory muscle disease. To be eligible, you must have tested positive for specific antibodies linked to your muscle condition, such as those found in dermatomyositis, immune-mediated necrotising myopathy, or anti-synthetase syndrome. If you use certain medicated skin creams, your dose needs to have been stable for at least four weeks before starting.

You cannot take part if your muscle condition first started during childhood, even if you are now an adult. You will also be excluded if your muscle disease is linked to a recent cancer diagnosis within the past three years. The study doctors will perform their own checks to confirm whether you qualify.

  1. I am aged 18 or older.
  2. I have been diagnosed with an active inflammatory muscle condition (IIM).
  3. My condition started in adulthood, not during childhood.
  4. I have not been diagnosed with cancer in the last 3 years.
  5. I am willing to attend regular hospital visits for drips, checks, and blood tests.

भागीदारी में क्या शामिल है?

Taking part in the study lasts for up to 112 weeks (just over two years) and is split into four stages. First, there is a screening period of up to six weeks to check that the trial is safe and suitable for you. Next is a 52-week period where you will receive either nipocalimab or a placebo as a regular drip into a vein. During this time, you will continue taking your normal muscle medicines, and neither you nor the doctors will know which study medicine you are getting.

After the first year, eligible participants enter a 48-week extension period where everyone receives nipocalimab. Finally, there is an 8-week safety follow-up after your last dose. Throughout the trial, you will attend hospital visits for physical checks, vital signs, and blood tests to monitor your health. Travel and parking costs for these visits can be reimbursed.

संभावित जोखिम और लाभ

There is no guarantee that taking part will improve your condition, as nipocalimab is still being tested and you may receive a dummy treatment during the first year. However, you will receive regular, close health checks from specialists, and your involvement could help improve future treatments for other people with muscle disease. Because nipocalimab lowers antibody levels, it can increase your risk of catching infections, reactivate dormant viruses, or make routine vaccines less effective. Other side effects may also happen. You are free to leave the trial at any point, and choosing to stop will not affect your usual NHS care.

सामान्य प्रश्न

प्लेसबो क्या है?

A placebo is an inactive dummy treatment that looks just like the real medicine. It helps researchers see whether changes in health are truly caused by the study drug or by other factors.

Will I have to stop taking my usual muscle medicines?

No. You will continue to take your regular muscle medications alongside the study treatment. If your condition stays stable or improves, your doctor may slowly lower your doses.

How is the study medicine given?

The medicine is given as a liquid drip directly into a vein in your arm during your hospital visits.

Do I have to pay to take part?

No, the study medicine and all related tests are completely free. You can also be reimbursed for reasonable travel and parking costs.

Can I leave the study early if I change my mind?

Yes, you can leave the study at any time without giving a reason. Withdrawing will never affect your standard medical care or your relationship with your healthcare team.
