Clinical trial to evaluate the correlation between variation in intestinal lactase activity measured with LacTEST 0.45g and alteration of intestinal barrier integrity and/or intestinal permeability in patients with celiac disease

Coeliac disease is a condition where eating gluten causes the immune system to damage the lining of the small intestine. When a person adopts a strict gluten-free diet, the gut usually begins to heal over time. A healthy gut lining produces an enzyme called lactase, which helps digest sugars found in milk. Gut damage often lowers the levels of this helpful enzyme. This clinical study is investigating an oral powder called LacTEST. Patients dissolve it in water and drink it. Researchers then measure how much of the broken-down sugar passes into the urine over five hours. By comparing these urine results with gut biopsies, blood tests, and stool samples over 12 months, researchers hope to see if LacTEST can reliably show whether the gut lining is repairing itself. This could offer a simpler, less invasive way to check gut recovery.

यह अध्ययन किस बारे में है?

Coeliac disease is an autoimmune condition where eating gluten—a protein found in wheat, barley, and rye—causes the body to attack its own gut lining. Over time, this flattens the tiny finger-like projections (villi) in the small bowel that help absorb nutrients. One of the key enzymes found on healthy villi is lactase, which helps break down dairy sugars. When the gut is damaged, lactase levels drop, and the gut barrier can become leaky.

The only effective treatment for coeliac disease is a lifelong gluten-free diet. As the gut heals, the villi grow back, lactase levels recover, and the gut lining becomes stronger. At present, the main way doctors check if the gut has truly healed is by performing an endoscopy—passing a small camera down into the gut to take tiny tissue samples (biopsies). Because endoscopies can be uncomfortable, doctors are looking for gentler ways to track healing.

This trial investigates a testing product called LacTEST. It is a powdered drink that contains a sugar compound broken down specifically by the lactase enzyme. If your gut is healing and producing lactase, the sugar is processed and later filtered out into your urine. By checking urine samples over five hours after having the drink, doctors can see how much enzyme activity is present.

Over the course of a year, researchers will track participants to see if the LacTEST urine results match up with standard gut biopsies, blood antibody tests, and stool checks. If proven accurate, this test could provide a much easier way to monitor gut recovery and barrier strength in people with coeliac disease.

मुख्य निष्कर्ष

  • Tests a simple oral drink to see if it can track gut healing in coeliac disease.
  • Open to adults aged 18 and over who have coeliac disease.
  • Follows participants over a 12-month period while on a gluten-free diet.
  • Involves collecting urine for five hours after drinking the test solution.
  • Compares urine results with routine blood tests, stool samples, and gut biopsies.

कौन पात्र हो सकता है?

This study is open to adults aged 18 and older who have been diagnosed with coeliac disease. Both men and women can take part.

Participants should be starting or following a gluten-free diet as part of their standard management plan. Because the trial tracks gut recovery, it is designed for people whose gut health can be observed over a 12-month period.

The study team will review your full medical history to make sure taking part is safe for you. If you have other bowel conditions or allergies that could interfere with the test results, the study doctor will discuss whether this trial is a good fit.

भागीदारी में क्या शामिल है?

Taking part in this study lasts for about 12 months. During this time, you will continue your normal gluten-free diet and attend scheduled clinic appointments, typically at the start, at 6 months, and at 12 months. At these visits, you will be asked to drink the LacTEST solution (a powder mixed with water) and collect your urine over the following five hours. You will also provide routine blood samples to check coeliac antibodies and stool samples to measure gut inflammation. Standard gut assessments, which may include an endoscopy to examine the gut lining directly, are carried out at the beginning and after 12 months to compare against the urine test results.

संभावित जोखिम और लाभ

Taking part may not provide any direct health benefit to you, but it gives you very close monitoring of your gut recovery while on a gluten-free diet. The findings could also help develop easier, non-invasive tests for others with coeliac disease in the future. The risks of the LacTEST drink are generally low, though some people might experience mild tummy rumble, gas, or mild digestive upset. Having blood taken may cause temporary tenderness or bruising, and standard endoscopies carry mild discomfort such as a sore throat. Your participation is completely voluntary, and you can withdraw at any point without it affecting your usual medical care.

सामान्य प्रश्न

What is LacTEST?

LacTEST is a harmless sugar powder dissolved in water. It is designed to be broken down by the gut enzyme lactase so doctors can measure how well your bowel is working through a urine sample.

Will I still need an endoscopy?

Yes, standard tissue samples taken during an endoscopy are used in this study to check whether the urine test results match the actual physical healing of your gut lining.

Do I have to change what I eat?

No, you will simply follow the standard gluten-free diet recommended by your healthcare team to allow your gut to heal.

How long will each test visit take?

Visits involving the LacTEST drink require urine collection over a five-hour window, so you will need to plan for half-day appointments on testing days.

अगर मैं अपना मन बदल लूँ तो क्या मैं अध्ययन छोड़ सकता हूँ?

Yes. You are free to stop taking part at any time, and doing so will not alter your normal relationship with your doctors or your treatment.
